Shelter + Care Case Manager

Full Time Days

** Salary starting at $44,000/year; based on experience **

Lutheran Social Services of Central Ohio is currently seeking a Shelter + Care Case Manager for Faith Mission in Fairfield County. Faith Mission is one of the largest homeless shelters in our area providing meals and shelter to men, women and Veterans with dignity and compassion.

What will I do as a Shelter + Care Case Manager with Lutheran Social Services?

  • Work with residents to develop action oriented individualized goal plans that support client growth in self-sufficiency and stability.
  • Establishes and maintains file, ensures timely, accurate, and complete case notes and documentation and statistics as required for the program including detailed documentation of meeting dates, referrals, case management hours, etc.
  • Implement planning systems that reflect resident strengths and needs, responsibilities, and action steps to help residents achieve their goals.
  • Coordinate with Faith Mission Resource Specialists to assist client in seeking or growing income, job training, education and/or employment as appropriate.
  • Gather and disperse to residents' information on potential community resources.
  • Maintain clear and precise data and case notes, including monthly budget reporting and careful documentation of all resident interactions.
  • Assists clients as needed with transportation to move into housing, grocery shopping, etc., by accessing Lancaster Transit and other options when possible.
  • Assist in housing search needs of new shelter plus care clients.
  • Ensure that residents are provided with a clean, safe, well-maintained community and that residents participate in maintaining the same.
  • Orientation to all new shelter plus care recipients.
  • Participate in month shelter plus care meetings and maintain documentation.

Requirements for a Shelter + Care Case Manager with Lutheran Social Services:

  • Bachelor's degree in social work or related field required or equivalent level of education and experience.
  • Previous experience working with homelessness, mental health or related services.
  • Experience working in Mental Health field required.
  • Valid Ohio driver's license, access to reliable insured transportation and ability to meet LSS insurance standards required.
