We need an RN (Registered Nurse) who specializes in Skin Integrity to join our team at Heritagespring Skilled Nursing and Rehabilitation in West Chester , OH .

Registered Nurse rate of pay: $35 to $41 Per Hour, PLUS  On top of those great base rates we offer additional shift differentials for evening and weekend shifts!!

Our Registered Nurses Will Be Responsible For:

• Completing weekly head to toe skin assessments & document in the EHR
• Ensuring that appropriate treatments & interventions are in place for skin issues identified during the skin rounds
• Collaborating with the Clinician or Wound NP to obtain new orders or discontinue orders as needed
• Updating the ADON & DON of changes in resident condition
• Updating the ADON & DON of any a non-healing wounds
• Verifying & updating resident care plans & documentation for identified skin impairments
• Monitoring skin integrity practices & procedures to ensure that personnel are implementing the facility’s SIT program
• Demonstrating proper infection control practices
• Assisting with evaluating & implementing recommendations from the Quarterly QA meeting
• Ensuring that at-risk residents & those with known non-compliance behaviors are provided with appropriate interventions
• Participating in weekly SIT meetings.
• Ensure residents’ responsible parties are updated regarding wound progress
• Developing positive employee relations & promoting teamwork, mutual respect & effective communication
• Educating new & inexperienced nurse aides in proper methods of patient care

You’ll Need To Have:

• A current RN license for the state in which you’re working
• Flexibility with your working schedule
• A thorough understanding of the integumentary system & a willingness to continue training
• A high degree of leadership, organizational ability & communication skills
• A desire to constantly update your knowledge through ongoing education
