JOB FUNCTION :
The Child Life Contractor is responsible for completing child life history assessments, as these documents are required for a child to be adopted. The Child Life Contractor requests and collects all relevant documents, interviews relevant persons, and completes the Child Life History assessments.
Core Responsibilities and Expectations - Reviews the childs DFCS file and available documents needed to complete the CLH checklist.
- Requests and collects supporting documents
- Completes requests for missing documents as needed to complete the CLH for medical, mental health, educational, etc.
- Confirms that record requests have been received by providers and gets clarification on timeline for release of records.
- Reads CCFA and makes notes of any further information that needs to be gathered.
- Reviews and documents placement history and request interview with DFCS worker if needed to understand reasons for change of placements, causes of disruptions, etc.
- Reviews parent psychological and makes notes of any further information that needs to be gathered.
- Reviews court orders.
- Interviews caregivers to collect information, child functioning, school history, mental health, etc.
- Documents interview of foster parents/caregivers in Extended Reach.
- Writes CLH narrative that includes (but not limited to): the childs description, personality, and current functioning, school experience/day care, psychological/psychiatric history, medical history, developmental history, placement history, birth family history, childs readiness for adoption, adoptive home recruitment plan if needed, a list of adoptive resources that are needed, and length of time estimated for the child to be emotionally ready to transition to an adoptive placement.
- Uploads all documents received to extended reach and finalizes CLH checklist.
- Ensures accuracy of all records, reports and documentation and maintains organized files.
- Maintains confidentiality of clients and agency information.
- Maintains timely communication with others and is able to effectively meets deadlines.
- Displays a strong discernment and decision-making skills.
- Displays effective, clear, concise writing skills.
- Maintains a spiritual growth and a relationship with Christ through devotions.
- Completes all continuing education requirements and all required trainings.
- Participates in the onboarding of new staff as needed.
- Completes other duties as assigned.

QUALIFICATIONS :
Skill Requirements:
Bachelor's or Masters degree from an accredited college or university in the area of behavioral or social sciences, social work, guidance counseling, psychology, related field.
Bachelors degree requires a minimum of 3 years experience in child welfare programmatic; Masters degree requires a minimum of one years experience.
PHYSICAL REQUIREMENTS: Hearing and speaking ability which allows for effective oral communication of information.
Ability to physically transport self and clients to various locations.
Physical and emotional stamina to effectively handle job related issues and stress.
SKILL REQUIREMENTS: - Strong interpersonal skills.
- Maturity and sensitivity to cultural and individual differences in clients, staff, foster parents, and families served.
